The "Lifestyle Management Clinic" is held every Thursday from 4 p.m. to 7 p.m. in
the Health Check Up premises (3rd Floor, Hinduja OPD Clinic Bldg)
1. To book your appointment, fill the form below. You will receive an
acknowledgment from us within 24 hours. We also consider walk in appointments.
Please note: Filling up of the form for appointment does not indicate confirmation
of an appointment. Appointment confirmation is subject to receipt of payment.
The hospital reserves the right to change or cancel the appointment without prior
notice.

2. As the next step after your registration, you are required to fill the STRAP
Questionnaire.
It is advisable to fill the form, in advance & bring a hard copy of the same on the day
of the appointment. In case you decide to fill the questionnaire on the day of your
appointment, please allow a margin time of 45 minutes prior to your appointment
slot to fill up the form.
In case of Online Payments:

Do not forget to bring the printout of the Receipt, ID proof and the Credit/
Debit Card used to book appointment(s) online on the day of appointment.
If the holder of the credit/debit card is not the person undergoing

Consultation, then the person should possess: s photocopy of both sides of
the credit/debit card, which will have to be self attested by the credit/debit
card holder authorizing the use of the credit/debit card for the purchase.
