This book is aimed at quickly updating the reader on osteosarcoma, a dreaded primary bone cancer. Progress in management of osteosarcoma has been slow after the evolution of chemotherapy and limb salvage surgery. Research is now directed towards identifying molecular targets for systemic therapy. Availability of chemotherapy drugs and low cost implants in developing world have allowed limb salvage surgery to develop. This book looks at current basic knowledge on osteosarcoma and some of the developments in research which have the potential to change the prognosis.

Ongoing Projects
Development of an indigenous modular titanium prosthetic system for reconstruction of tumor related defects.
This one million dollar government funded project is being done jointly with engineers from the Indian Institute of Technology (I.I.T, Mumbai) and Non Ferrous Technology development corporation (NFTDC, Hyderabad). A new design as per international and ISO standards is being formulated as an improvement over the existing indigenous prosthesis, and a prototype has been made. A Knee simulator is also being made indigenously for testing these designs. Instrumentation is also being simultaneously designed. Software for preoperative planning and virtual simulation of implantation is also being developed. A permanent lab has been constructed in the I.I.T Mumbai campus for this “Ortho-CAD” project. HA coating and HDPE machining have been the technologies which have been developed. The aim is to provide high quality customized and modular reconstruction implant systems using all modern technology at affordable prices. Clinical trials with the new prosthesis are expected to start by Jan 2010. .
"Evaluation of Zolendronic acid as a single agent and as an adjuvant to chemotherapy in the management of high grade osteosarcoma".
Bisphosphonates have been shown to be tumoricidal in osteosarcoma cultures and in animal studies. Zolendronic acid has been shown in laboratory studies to be toxic to osteosarcoma cells. In combination with chemotherapy drugs like paclitaxel, it has been shown to enhance response in animal studies. It is postulated that Zolendronic acid could improve the response to chemotherapy in humans (measured at pathology by a percentage of tumor cells found dead) and also the disease free and long term survival by reducing the metastatic disease. This study is designed as a preliminary phase II study to determine whether there is any significant activity of Zolendronic acid in osteosarcoma. Recruitment has now begun
"Evaluation of curcumin formulation, and Ashwagandha extract in the management of advanced osteosarcoma"
Curcumin and Ashwagandha are ingredients from traditional Indian medicine with demonstrated safety of use. Curcumin is an extract of turmeric (`Haldi’), which is a common ingredient in Indian cooking. It has antitumor, antioxidant, antinflammatory activity with practically no pharmacological toxicity. Similarly Ashwagandha (Withania Somnifera) is a plant found in abundance all over India. It has been used in several Ayurvedic preparations for arthritis. It is also known as the “Indian Ginseng” due to its anti-oxidant and cell protection properties. It has been shown to be active in several tumor cell lines including the lung, breast, and colon. In animal experiments it has been shown to reduce the toxicity of chemotherapeutic agents like Paclitaxel. Toxicity studies have shown it to be safe over prolonged period of administration. Both these substances seem to show promise in laboratory and animal studies of cancer. A scientific study is required to find out if any of them have the potential of being useful as adjuvant in osteosarcoma. This study is designed as a preliminary proof of concept study to determine whether there is any significant activity of these substances in osteosarcoma. Recruitment is on.
"Efficacy of Pasteurization of resected tumor bearing bone and immediate reimplantation in surgical treatment of malignant bone tumors."
Pasteurization has been reported as a clinically used method from Korea in small series. In this project we will prove the efficacy of pasteurization in destroying all mitotic activity of tumor by clonogenic studies as well as animal inoculation and also do a clinical pilot study to set up procedure and assess clinical efficacy. We hope to show that this method will be superior to extracorporeal irradiation and reimplantation. The project has been recently approved by IRB and funded and will begin shortly.
Preclinical evaluation of osteointegration of titanium diaphyseal implant "grown" on a metal rapid prototyping machine and transplanted with mesenchymal stem cells in animal model.
In this project we will study whether a diaphyseal implant “grown” on a metal rapid-prototyping machine by laser sintering in layers will stimulate adequate bone ingrowth in a rabbit model. We will also assess whether coating with mesenchymal stem cells will improve bone formation on the implant. 3cm of rabbit tibia will be replaced with this special implant with a porous surface and bone ingrowth studied after a specified time period. The project has been recently approved by IRB and funded and will begin shortly.
